General Liability Waiver Release — The Borough of Jefferson Hills In consideration of being permitted to use/participate in the Borough of Jefferson Hills' fields/facilities/property/activity, the visitor/guest, its personal representative, heirs, assigns, hereby acknowledge, agree, represent, and warrant: Visitor/guest will observe all safety, hygiene and sanitation guidelines set forth by federal, state, and local authorities, including, but not limited to, the Centers for Disease Control, the Pennsylvania Health Department, the Allegheny Health Department, the Borough of Jefferson Hills, law enforcement, and other authorities issuing reasonable guidance in the prevention of infectious disease. Visitor/guest assumes full responsibility for and risk of bodily injury, illness, death or property damage (whether due to ordinary negligence or otherwise) arising in connection with use/participation in the Borough's fields/facilities/property/activity. Visitor/guest hereby releases, waives, discharges and covenants not to sue the Borough of Jefferson Hills from, against, or for any and all liability for loss or damage, and any claim or demands on account of personal injury, illness (including, without limitation, viral infection, including COVID-19), or death, or damage or destruction of property, in each case, regardless of cause, arising in connection with use/participation of the Borough's fields/facilities/property/activity. Visitor/guest shall be liable and shall indemnify the Borough of Jefferson Hills against any and all injury or loss resulting from visitor's/guest's acts. Visitor/guest understands that this agreement is intended to be as broad and inclusive as permitted by the laws of the United States of America and the State of Pennsylvania and agrees that if any portion is held invalid, the remainder of the Agreement will continue in full legal force and effect.

I have read and agree to the Player Verification Waiver *

(I/We), the parent(s)/guardian(s) of the named registrant(s) for the TJ Youth Baseball program, hereby give approval for the participation in any and all sponsored activities during the current season. I assume all risks and hazards incidental to such participation including transportation to and from activities, and hereby waive, release, absolve, indemnify and agree to hold harmless the TJ Youth Baseball Association, its board, sponsors, supervisors, web hosts, web developers, participants, and persons transporting the participant to and from activities for any claim arising out of any injury which may occur. I will furnish a certificate of birth for the above named registrant upon request of the league officials. I understand that if for any reason the above named participant should not be able to participate in the program, the registration fee is NON refundable. EXCEPTION: Prior to being placed on a team, the following are examples of acceptable exceptions; prolonged illness, disabling injury, moving out of the borough. Further, I authorize TJ Youth Baseball, and its officers, to photograph the named registrants, and display their names in compliance with the TJYBB rules and acknowledge that all online content becomes the property of and its web host to be used for the programs and advocacy efforts of the organization.
